About The Position

Destiny Healthcare Services is a reputable and growing healthcare agency in the North Chicago land area, and we are seeking a compassionate Registered Home Health Nurse to join our team. As a Registered Home Health Nurse, you will provide in-home skilled nursing care to patients, helping them stay healthy and independent in the comfort of their own homes. This role requires exceptional clinical skills, strong communication and critical thinking abilities, and a dedication to patient care. Compensation & Benefits: We are currently looking for a part-time nurse and are paying 100 dollars per client visit! We offer competitive compensation for this position that pays per visit. Requirements

• Active and unrestricted RN or LPN license in the state of Illinois
• Preferably 1 year of nursing experience, preferably in home health or a similar setting
• Strong clinical skills and knowledge of medical terminology
• Ability to work independently or with others and make critical decisions
• Excellent communication and interpersonal skills
• Current CPR certification
• Must have a reliable means of transportation and a valid driver's license

Responsibilities

• Conduct initial and ongoing assessments of patient's medical conditions and home environments, and collaborate with the care team to create individualized care plans
• Administer medications and treatments as prescribed by the physician
• Provide wound care, catheter care, IV therapy, and other skilled nursing services
• Educate patients and their families on disease management, self-care, and medication management
• Document patient progress, interventions, and outcomes in a timely and accurate manner
• Collaborate with other healthcare professionals and community resources to coordinate patient care
• Communicate effectively with patients, families, and caregivers to ensure their needs are met
• Adhere to all HIPPA, and state/federal regulations and guidelines
