Requirements

Current Registered Nurse (RN) license in the state of Michigan.
Minimum of 1 year nursing experience preferred, especially in home health or related fields.
Strong clinical assessment and critical thinking skills.
Excellent interpersonal and communication skills.
Ability to work independently and manage time effectively.
Reliable transportation to travel to patient homes in Taylor, MI and surrounding areas.
Basic computer skills for documentation purposes.

Responsibilities

Provide skilled nursing care and health assessments to patients in their homes.
Develop and implement individualized care plans based on patient needs and physician orders.
Monitor patient conditions and report changes to the healthcare team promptly.
Administer medications, treatments, and therapies as prescribed.
Educate patients and their families on health management, disease prevention, and post-care instructions.
Maintain accurate and timely documentation of patient care activities.
