About The Position

The Registered Dietitian at Sharecare is responsible for educating patients with diabetes in an outpatient setting, collaborating with healthcare professionals to enhance clinical outcomes and patient satisfaction. This role involves assessing patients' understanding of diabetes management, providing education, and implementing interventions to improve disease management.

Requirements

  • Registered Dietitian required.
  • Certified Diabetes Care and Education Specialist (CDCES) or Board Certified-Advanced Diabetes Management (BC-ADM) preferred.
  • Minimum 3 years of related experience in a hospital or physician office setting preferred.
  • Appropriate professional licensure/registration required.

  • Experience in a digital health environment.
  • Knowledge of current diabetes management guidelines.

Responsibilities

  • Partner with the Program Director, physicians, and hospital staff to achieve program objectives and enhance clinical outcomes.
  • Assess patients' understanding of diabetes and assist them in setting self-care goals.
  • Provide group and individual education on diabetes management and medical nutrition therapy.
  • Implement interventions such as self-blood glucose monitoring and insulin pump therapy as certified.
  • Work with referring providers to identify diabetes management needs of patients and families.
  • Collaborate with an interdisciplinary team for case management of complex diabetes patients.
  • Assist in marketing and community services as needed.
