About The Position

Schedule: Temporary role, full-time 40 hours per week | Monday - Friday, 8:00am - 4:30pm Hybrid role, can be flexible with office days and work from home days. Department: Thome PACE | Jackson, MI What You'll Do: In this temporary position, you’ll assess participants’ nutritional status, design and implement personalized care plans, and determine precise nutrient needs. You’ll play a key role in recommending targeted nutrition therapies, monitoring progress, and evaluating outcomes to ensure the highest quality of care. Collaborating closely with physicians and interdisciplinary teams, you’ll share valuable insights that drive better health results. As a Registered Dietitian, you’ll grow your skills and make a difference while working with the IDT at Thome PACE.

Requirements

  • Active status as a Registered Dietitian Nutritionist (RDN) with the Commission on Dietetic Registration (CDR) and Academy of Nutrition and Dietetics (AND).
  • Bachelor’s Degree in Nutrition or Dietetics from an Academy of Nutrition and Dietetics (AND) accredited institution
  • Must be CPR certified or willing to obtain.
  • Minimum of 1 year of experience working with the frail elderly population or in a similar healthcare setting.
  • Must have reliable transportation and possess a valid and current driver's license.
  • Ability to teach and educate participants and staff on nutrition-related topics.

Responsibilities

  • Conducts comprehensive nutritional assessments and reassessments.
  • Identifies need and implements appropriate nutrition services, which may include home delivered meals and nutritional supplements.
  • Provides personalized nutrition education to participants and caregivers.
  • Delivers nutrition education to staff and the community through wellness programs, seminars, and outreach activities.
  • Collaborates with the IDT to integrate participant education and care plans effectively.
  • Accurately documents changes in participants’ nutritional status in the medical record and communicates updates to relevant team members.
  • Assists in monitoring meal services, including verifying diet compliance, meal intake, and tray accuracy.
  • Conducts regular and random inspections to ensure proper sanitation and cleanliness of food preparation and kitchen areas.
  • Actively participates in Quality Improvement (QI) processes and contributes to improving nutrition-related outcomes.
  • Serves as a vital member of the Interdisciplinary Team (IDT).
