As a part of Uptown Community Health Center, Bruner Family Medicine sees patients of all ages and backgrounds. As the main clinic for a Residency program we have over 30 different Providers. We have 4 specialty clinics within Bruner that are ran by our Faculty. Summary The Registered Dietician/Nutritionist supports the Health Center by providing nutritional assessment, education and management to patients with chronic conditions who are at risk for disease progression, complications, and functional decline. In collaboration with the physician and interdisciplinary team, the Register Dietician (RD) develops and implements an individualized care plan designed to promote the patient's understanding and management of their condition, optimize quality of life, and improve clinical outcomes. Essential Functions Upholds the Mission, Vision, and Values of Uptown Community Health Center. Be available to work in person as scheduled and report to work on time regularly and reliably. Maintains a safe, secure, and healthy environment by adhering to organization standards, policies, and procedures, and to legal regulations; alerting others with potential concerns. Communicates in a professional, clear, effective, and compassionate way with patients, visitors, and coworkers. Both in person, on the telephone, and with written communication. Assesses the health needs and diet of patients based on provider referral RD services. Develops, implements, and documents patients care plans. Counsels patients on nutrition issues and healthy eating habits. Develops meal plans, taking both cost and healthy eating habits with the patient. Evaluates the effects of meal plans and change the plan as needed with the patients and/or family. Collaboratively develops an individualized follow-up plan with each program participant. Promotes better nutrition by speaking to groups about diet, nutrition, and the relationship between good eating habits and preventing or managing specific diseases. Acts as a nutrition liaison/resource to other departments within the organization and the community. Evaluates effectiveness of educational services provided by measuring attainment of learning objectives. Documents assessment data, educational plan, educational services provided and evaluation results in each participant's educational records. Utilizes a team approach to provide services and collaborates and communicates with team members when needed. Identifies when a program participant's needs are outside the scope of the instructor's practice and expertise, plus arranges additional services to meet needs. Proper and accurate documentation within the electronic medical record: recording patient examination, treatment, and test results. Maintains patient confidentiality and protects medical office operations by keeping information confidential; cautioning others regarding potential breaches. Maintains a clean work area: clean and stock the rooms in accordance with the Kanban system. Attends required staff meetings, standard work meetings, and trainings. Keep up with latest nutritional science research and maintain resource aids. Other duties as assigned
