About The Position

The Registered Dietitian Nutritionist will maintain ADCES accreditation while providing education, treatment, counseling, and case management for American Indian/Alaska Native (AI/AN) patients with or at risk for diabetes. The role focuses on diabetes care, education, and prevention strategies, as well as weight loss support for patients on GLP1 Therapy.

Requirements

  • Valid Registered Dietitian license is required.
  • Bachelor's degree in a relevant field is preferred.
  • Experience in weight loss counseling is preferred.

  • Familiarity with GLP1 Therapy.

Responsibilities

  • Manage specified diabetes clinics ensuring smooth patient flow and monitoring of diabetes complications.
  • Assist with reporting metrics for wellness programs in coordination with the Contract and Grants Program Manager.
  • Provide diabetes self-management care and education to patients with or at risk for diabetes.
  • Instruct patients in the use of glucometers and continuous glucose monitors, and counsel on maintaining appropriate blood glucose levels.
  • Teach weight management strategies and provide ongoing support to patients.
  • Provide medical nutrition therapy including assessment, education, and intervention on various metabolic disease states.
  • Create patient education materials using Canva and coordinate nutrition workshops for community members.
  • Collaborate with a multidisciplinary team to develop patient care plans and provide case management.
  • Document patient care and education in the electronic health record.
  • Review and recommend updates to policies, procedures, and communication materials.
  • Prepare monthly reports and ensure adherence to client confidentiality.
  • Participate in community health promotion events and assist with prevention projects.
