About The Position

Provide culturally appropriate nutrition education and medical nutrition therapy (MNT) to low-income, underserved patients of St. Vincent de Paul that struggle with or are at risk for chronic diseases such as obesity, pre-diabetes, diabetes, and associated co-morbidities. Coordinate and provide a full range of nutritional care services across the lifespan that promote health and supports patients in achieving health care goals. The RDN – Level 1 is an entry-level practitioner (or an experienced RDN transitioning to a new practice focus) who delivers services consistent with professional standards of dietetics practice. The role builds proficiency through supervised experience, on-the-job training, and continuing education, including diabetes care competencies as applicable Performance expectations for this role are aligned with the ICFW Professional Practice and Leadership Framework, which guides annual evaluations, professional development planning, and advancement within the RDN career ladder.

Requirements

  • Bilingual: Spanish and English.
  • Valid Driver’s license and reliable personal transportation.
  • Ability to work evening shifts 2-3 times per week as needed
  • Master’s degree required for individuals completing CDR eligibility requirements in 2024 or later (unless grandfathered under prior CDR standards).
  • Completion of an ACEND-accredited Didactic Program in Dietetics (DPD) and eligibility to sit for the Commission on Dietetic Registration (CDR) exam.
  • Pass the Commission on Dietetic Registration's dietetic registration exam within 3 months of employment. Maintain credentials through the duration of employment.
  • Ability to provide proof of successful completion of an ACEND-accredited dietetics internship program
  • Able to stand, walk, sit, bend, reach and lift up to 30 pounds are required for this position.

Responsibilities

  • Provides direct patientcare using medical nutrition therapy (MNT),diabetes self-management (DSME) and diabetes prevention education (DPP) to individuals and families.
  • Implements a nutritional care plan, communicates plan, establish ESA follow-up system and maintains documentation for patient visits as outlined in ICFW policies and procedures.
  • Coordinates care plan with other health care disciplines and departments to optimize nutrition and diabetes care.All diabetes management care plans will be implemented under the supervision of a Certified Diabetes Care and Education Specialist.
  • Initiate referrals when indicated or outside of scope of practice to appropriate providers and/or programs.
  • Bases intervention/plan of care on best available research/evidence and information, evidence-based guidelines, and best practices.
  • Facilitate services through individual interaction, groups session, in-person (on-campus and off-campus locations) and/or through distance learning.
  • Coordinate and/or participate in ICFW education programs, as assigned by ICFW Director.
  • Develops and contributes to ICFWs culturally appropriate and evidence-based education programs to prevent chronic disease and health promotion that target vulnerable and at-risk groups of people.
  • Create and revise educational materials appropriate forage, literacy level, cultural background, and physical and cognitive abilities of recipients.
  • Actively participates in program development and evaluation including outcomes reporting that informs stakeholders and potential fundin gopportunities.
  • Support training of Community Health Educators, volunteers, and interns in ICFW service delivery with information that is current, accurate, and evidence based.
  • Work with community stakeholders and ICFW staff to achieve ICFW program goals and continuous quality improvement.
  • Establish and participate in outreach and promotion activities to expand the reach of services and increase the enrollment rate into educational offerings.
  • Complete required organizational training, refresher or new skills training, and training offered by the ICFW.
  • Comply with all applicable laws and regulations, including those governing participant privacy and data security (e.g., the Health Insurance Portability and Accountability Act [HIPAA])
  • Participate in and serve as a model for SVdP’s Culture of Philanthropy, and ensure SVdP volunteers feel welcomed and appreciated Volunteer engagement and appreciation, as well as Culture of Philanthropy participation are part of all job descriptions at SVdP, and a very important part of the organizational culture and values
