About The Position

The registered dietitian provides medical nutrition therapy to ambulatory patients individually or in a group setting. The registered dietitian provides a nutrition assessment, determines and implements a nutrition care plan and intervention. Develops and implements nutrition education programs for the general public, community organizations and for medical and clinic staff.

Requirements

• Bachelor's Degree in a relevant field
• Licensed Dietitian Nutritionist - Illinois Department of Financial and Professional Regulation (IDFPR)
• Registered Dietitian (RD) - Commission on Dietetic Registration (CDR)
• 9+ years of experience as a Dietitian
• Possesses planning, leadership and organizational skills
• Demonstrates ability to accept and institute change
• Communicates effectively and works collaboratively with others

Responsibilities

• Provide nutrition services and medical nutrition therapy integrated with the patient's medical goals
• Identify and provide age-specific nutrition counseling to meet the cultural needs of the patients and their families
• Evaluate, interpret, monitor and document the nutritional status and nutritional needs of patients using established standards of care and practice guidelines
• Develop, review, update and implement educational materials to meet the needs of patients and professionals
• Completes clinical and educational assessment on all patients
• Develops individualized teaching plans for self-management (Diet, Medication, Self-care)
• Implements evidence based clinical practice guidelines for all area of nutrition education
• Provides education to all patients utilizing insulin pump
• Collaborates with all members of the health care team
