About The Position

The Registered Dietitian, under the direction of the Director of Community Health and Well-Being, provides clinical nutrition services to Food Farmacy patients by providing medical nutrition therapy, performing all components of the Nutrition Care Process (including nutrition assessment, nutrition diagnosis, nutrition intervention, and nutrition monitoring and evaluation). Ensures patient satisfaction, quality care, regulatory agency compliance, and good public relations are achieved through the safe and efficient use of resources.

Requirements

  • Bachelor’s degree in Food & Nutrition, Dietetics, or related field from an accredited college or university.
  • Registered Dietitian/Nutritionist.
  • Previous diabetes education experience preferred.
  • Excellent verbal and written communication skills.
  • Ability to communicate effectively with patient, family, and members of the health care team.
  • Ability to calculate figures and amounts such as percentages and apply basic algebraic concepts.
  • Possesses necessary skills to effectively operate the hospital and department computer-based systems including ability to operate a standard computer, and basic use of application software such as Excel, Word and PowerPoint.

Responsibilities

  • Assesses nutritional status of patients and identifies nutrition problems/diagnoses as appropriate.
  • Develops and implements nutrition care plans utilizing appropriate nutrition interventions (including oral diets, referral to other services/community agencies).
  • Monitors the effectiveness of nutrition interventions, evaluates patient outcomes, and revises nutrition care plan as needed.
  • Educates/counsels patients/families individually or in groups on therapeutic diets and nutrition principles.
  • Communicates/collaborates with members of the Food Farmacy/health care team regarding nutrition care plans.
  • Completes all required documentation, reports, and productivity logs in an accurate and timely manner.
  • Orients/trains/mentors/precepts students, interns, staff, and other professionals.
  • Participates in department and organizational performance improvement/quality activities/initiatives, research, and assigned committees, councils, and/or task forces.
  • Assists Food Farmacy CHW with other aspects of the program including designing recipes and advising on Food Farmacy food utilization.
  • Represents the Community Health and Well-Being department by responding to media requests, and participating in outreach activities.

  • Pay based on experience and location: $29.00-41.40
