The RD – Food as Medicine (FAM) Coordinator leads the design, implementation, and coordination of TrueCare’s comprehensive Food as Medicine strategy across clinical and community settings. This role supports the integration of evidence-based nutrition and food access interventions such as medically tailored groceries and meals, produce prescriptions, food pantry services, teaching kitchens, and nutrition education into patient care to improve health outcomes and advance health equity. The Coordinator works collaboratively with clinical teams, care management, community partners, and vendors to operationalize Food as Medicine programs, ensure culturally responsive patient engagement, and align initiatives with value-based care goals. This role provides program oversight, coordination, and technical support across multiple sites, with a focus on improving food security, nutrition literacy, and chronic disease prevention and management for underserved populations Your Role & Impact As a Registered Dietitian FAM, you’ll partner with patients of all ages to deliver culturally responsive nutrition education and Medical Nutrition Therapy (MNT). You’ll support chronic disease management, collaborate with interdisciplinary teams, and help patients set and achieve personalized health goals through motivational interviewing and evidence-based care.

Requirements

  • Bachelor’s degree in food and nutrition services, nutrition sciences, dietetics, or related field.
  • Current accreditation by the Commission on Dietetic Registration.
  • A minimum of three (3) years of experience as a dietitian in community nutrition or public health.
  • Knowledge of general, therapeutic and community nutrition.
  • Knowledge and experience in nutrition education and interventions.
  • Knowledge of Food as Medicine initiatives, programs, and services.
  • Strong organizational and data management skills including collecting, reviewing, and analyzing survey data.
  • Computer proficiency, particularly with electronic health/medical record systems such as Epic and the Microsoft Office suite, including Outlook, Word, Excel, and PowerPoint.
