About The Position

The position is for a full-time Dietitian Nutritionist at Advocate Health, specifically within the Internal Medicine Residency program. The role involves providing diabetes self-management education and medical nutrition therapy to patients and their families. The Dietitian will conduct assessments, educate patients, implement teaching plans, and collaborate with healthcare teams to ensure effective diabetes management. The position requires maintaining knowledge of current trends in nutrition therapy and diabetes management, developing educational materials, and participating in professional organizations.

Requirements

  • Bachelor's degree in Dietetics.
  • 1-3 years of experience in clinical specialty.
  • Current licensure in the State of Illinois as a registered dietitian.
  • Previous diabetes education experience.
  • Ability to communicate and work cohesively with diverse groups.
  • Excellent organizational and planning skills.

Responsibilities

  • Provide diabetes self-management education and medical nutrition therapy to patients and their significant others.
  • Conduct and document individualized assessment of pertinent medical, psychosocial, and educational history of patients.
  • Educate patients and their families in diabetes and nutrition education needed per visit type.
  • Implement and document teaching plan, educational interventions, and follow-up plan based on the individualized assessment.
  • Collaborate with patients and their families in establishing mutual learning goals and behavior change goals.
  • Document evaluation of outcomes and revise interventions and follow-up plan.
  • Assure education is based on current program curriculum and ADA guidelines.
  • Communicate education and follow-up recommendations to physicians and other healthcare providers.
  • Serve as a resource in diabetes management and nutrition education for the healthcare team.
  • Participate in educational programs regarding diabetes and nutrition.
  • Maintain knowledge of current developments in nutrition therapy and diabetes management.
  • Develop educational materials and displays.
  • Serve as a resource person and mentor to staff related to clinical knowledge of diabetes management.
  • Collaborate with physicians in planning or implementing research findings into nursing protocols.
  • Participate in professional organizations related to the subspecialty.
