About The Position

The Registered Dietitian will oversee the nutritional well-being of our clients. The dietitian will be an active member of each client’s Interdisciplinary Team (IDT) and provide ongoing assessments of each client’s nutritional status including identification of at-risk clients and creating diet plans to address these risks. The Dietitian will collaborate with Program Managers, nurses, Medical Director, medical providers, and other therapists to tailor dietary services for each client with the goal to improve health outcomes.

Requirements

  • Active license from the state of Tennessee as a Licensed Dietitian/Nutritionist
  • Must have a valid driver’s license
  • Good oral, written, and interpersonal skills to effectively interact with others.
  • Ability to maintain confidentiality.
  • Ability to follow procedures, instructions, laws, and policies
  • Ability to project a positive and professional image on behalf of the organization.
  • Must be an effective team member.
  • Ability to prioritize and manage time effectively.
  • Ability to effectively communicate with medical providers in the community concerning the dietary needs of clients.

Responsibilities

  • Treat clients with dignity, in a professional and considerate manner.
  • Provide dietary services in accordance with Open Arms Care guidelines and CMS regulations.
  • Provide timely annual dietary assessments and recommendations for each client to the interdisciplinary team. Provide as-needed assessments as identified by the interdisciplinary team.
  • Provide initial Dietary Assessment within 21 days of any new client admission.
  • Provide clients and Direct Support Professionals with education/training on the dietary needs of clients and general dietary guidelines as requested.
  • Monitor client weights for significant gains or losses and provide recommendations to the interdisciplinary team to address these concerns.
  • Monitor clients for dietary needs and health-related concerns regarding dietary needs, reporting health concerns to the Circle of Support and Medical Director.
  • Be an active participant of the interdisciplinary team – attending meetings and presenting dietary assessments and recommendations as requested.
  • Provide each home with menus for mealtimes including client-specific needs, which follow medical orders.
  • Interacts with client’s family members and/or conservator to maintain a positive relationship and ensure they are kept informed of any dietary needs of the client.
  • Must attend all required training/meetings and remain current on all required Open Arms Care certifications.
  • Be familiar with CMS regulations as these apply to an ICF/IID.
  • Other job duties as assigned.
