About The Position

Responsible for developing, implementing, and collaborating in multidisciplinary team-based approach between clinical services and community-based nutrition programs.

Requirements

  • Bachelor's Degree in Dietetics, Nutrition, or related field
  • Credentialed as a Registered Dietitian (RD) and licensed in the State of Texas
  • Minimum of three (3) years’ work experience in clinical setting, public health, or community nutrition
  • Must possess a valid Texas Driver’s License and be insurable
  • Must be able to successfully pass a post-offer drug screen and criminal history background check
  • Knowledgeable of medical nutrition therapy principles and concepts
  • Knowledge of nutrition across the life span
  • Knowledge of public health concepts, food safety and quality standards
  • Knowledge of human behavior and methods for effecting behavior change
  • Knowledge of population trends and community assessment
  • Ability to communicate effectively
  • Ability to work independently and as part of a team-based approach
  • Must have good organizational skills
  • Must have good time management skills
  • Must be familiar with nutrition related ICD-10, CPT, and Healthcare Common Procedure Codes
  • Must have Microsoft Office applications skills
  • Must be able to communicate effectively in both written and verbal form
  • Must have good analytical skills
  • Ability to maintain confidential client information
  • Ability to obtain BLS/CPR and Food Handler’s permit
  • Must be culturally sensitive to minority (especially Native American) populations

Responsibilities

  • Conducts comprehensive nutrition assessments and provides individualized counseling for clients with complex health conditions
  • Develops and implements evidence-based nutrition interventions, including individualized meal plans, to address chronic disease and health risks
  • Monitors and evaluates client progress, providing follow-up care and communicating nutrition status and recommendations to referring providers
  • Document encounters in the patient Electronic Health Record (EHR) software to meet standards for billing on a timely basis (e.g., ADA, CMS)
  • Serves as a subject matter expert on multidisciplinary teams, supporting coordinated nutrition strategies for prevention and disease management.
  • Identifies nutrition-related trends and risk factor data which informs service development, early intervention strategies, and program improvement.
  • Collects and documents data as necessary for reports or grant requirements
  • Participates in outreach and engagement efforts to promote services
  • Practices cultural sensitivity by respecting cultural nuances of traditional foods into nutrition recommendations whenever possible
  • Other duties as assigned.
