Role Scope: We are looking for a dynamic, service-oriented dietitian who is passionate about supporting elderly persons in their journey. The Dietitian will work within a multidisciplinary care team led by a primary care provider to ensure whole-person, integrated care. The Dietitian completes participant nutritional assessments and reassessments, screens and assesses participant’s nutritional status and uses pertinent data to plan and implement appropriate nutrition interventions, obtains participant food preferences and makes dietary modifications as needed, in addition to providing nutrition education to participants, caregivers and Habitat PACE staff. Our Sacramento PACE Dietician will set the vision for our overall medical nutrition program for our participants.

Requirements

  • Bachelor’s degree in food science, nutrition, and/or dietetics
  • Current medical terminology and the ability to complete written assessments and document participant care.
  • Certification by CDR as a registered dietitian
  • A minimum of one year experience working with the frail or elderly.
  • Proof of medical clearance for communicable diseases, including a TB test.
  • Proof of all immunizations up to date.

Nice To Haves

  • 2+ years as a practicing dietitian
  • Bilingual: Spanish/Mandarin/Cantonese preferred.
  • PACE experience and/or ADHC/CBAS experience
  • A state issued driver’s license, personal transportation, and auto insurance as required by law

Responsibilities

  • Contribute to a center experience that Participants want to spend time in, a team culture that cares and creates joy, and an environment where all participants and team members belong.

  • Develops and implements nutritional plan of care for participants that takes into account social circumstances and medical needs, in collaboration with the PCP and interdisciplinary team.
  • Plans and executes follow ups and reassessment for each participant, or as necessary if there are changes in participant status.
  • Assesses participants’ access to food and makes referrals to interdisciplinary team for meal preparation evaluation and grocery shopping needs.
  • Determine participants’ need for and order liquid nutritional supplements accordingly.
  • Provides instruction to participants and caregivers regarding dietary regimens and nutritional requirements (home visits may be necessary).
  • Develops and implements the nutrition care plans for high-risk participants including medical nutrition therapy recommendations.
  • Monitors food intake, therapeutic diets and special dietary modifications and communicates information and concerns with the care team.
  • Continue to raise the bar. Constructively seek and share feedback and help us implement changes in order to improve clinical outcomes and experience for participants.
  • Conducts sanitation and safety checks for kitchen and kitchen equipment as needed.
  • Monitors adequacy of lunch and snack menus and notes/makes adjustments to any dietary regimens or nutritional requirements based on individual participant’s needs.
  • Updates the lunch list, supplement sheet, diet lists and assisted dining room guide as needed.
  • Attends care planning sessions and reports on nutritional status of participants.
  • Works with the State Food Program as guided by state rules and regulations.
  • Serves as subject matter expert for the development of dietary program standardization, evaluation and team education.
  • Serve as subject matter expert to support quality programs and discipline-specific audits.
