About The Position

This position provides support and training to our MDS Coordinators to assess and monitor proper treatment for residents in nursing homes. The role involves overseeing the health status, care plans, and procedures for the intake of residents according to state and federal standards.

Requirements

  • Bachelor’s degree in Nursing or a related field.
  • Current RN license in good standing.
  • Minimum of 5 years of experience in MDS coordination or a related field.
  • Strong knowledge of state and federal regulations related to MDS and PDPM.
  • Excellent communication and leadership skills
  • Must possess the ability to make independent decisions, to follow instructions, and to accept constructive criticism.
  • Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the public.
  • Must maintain the care and use of supplies, equipment, etc., and maintain the appearance of maintenance work areas.
  • Ability to seek out new methods and principles and be willing to incorporate them into existing maintenance practices.

Responsibilities

  • Review, train, strategize, and collaborate with facilities' staff and co-resources to improve Quality Measures
  • Provide oversight on facilities’ weekly PDPM meetings for skilled patients
  • Track and monitor each building’s effective utilization of PDPM tools, including but not limited to Physician Diagnosis Verification, Admission/Discharge CSR(GG), Weekly CSR, PDPM Weekly Tracking Tool, and Point Right.
  • Create meaningful and effective clinical reporting systems between RDO and RDCS
  • Hold Regional MDS workshops monthly on PDPM and other CMS-related regulations
  • Track and monitor QM and QASP performance monthly and quarterly, providing data analysis and clinical recommendations to the facility for optimum outcomes.
