About The Position

Join our dedicated hospice care team as a Regional RN, where every shift you work directly supports patients and families with dignity, comfort, and care during life's most meaningful moments. We walk alongside patients and families, offering expert, compassion-centered hospice care. As a Regional Case Manager RN, you'll bring vital nursing care to multiple teams and locations, helping ensure seamless coverage and continuity of care across our service area.

Requirements

• A licensed RN with a valid [state] nursing license in good standing
• 2+ years of nursing experience
• Flexible, adaptable, and confident working in a variety of patient care environments and geographic areas
• Passionate about delivering high-quality care with dignity, kindness, and integrity
• Valid driver's license and reliable transportation (travel between locations is required)

Responsibilities

• Float between hospice care teams and locations, including patient homes, long-term care facilities, and our inpatient hospice home.
• Provide skilled, compassionate nursing care to patients and families at the end of life.
• Collaborate with interdisciplinary teams to follow and implement individualized care plans.
• Step in to support various teams during high-volume periods, staff absences, or urgent needs.
• Educate and guide families and caregivers during a vulnerable time with empathy and professionalism.
