The Regional Healthcare Manager (RHCM) is responsible for developing a network of referral sources, performing on-site evaluations of prospective Gaylord Hospital patients, facilitating the admissions process and communicating Gaylord's value proposition with healthcare organizations, referring healthcare practitioners, family members and insurance companies and meeting referral and admissions goals.

Requirements

  • ASN required; BSN or higher preferred.
  • Current CT RN or LPN license required.
  • Valid CT Driver's license required.
  • 4 or more years of RN or LPN experience in the healthcare field required, including 2 years in rehabilitation or insurance case management required.
  • Clinical assessment skills and knowledge of reimbursement issues impacting inpatient rehabilitation required.
  • Understanding and working knowledge of InterQual and NALTH criteria used for admissions preferred.

Responsibilities

  • Evaluates prospective patients referred for admission to Gaylord Hospital.
  • Receives referral information via phone or electronic information.
  • Performs on-site/remote evaluation, including completion and submission of pre-admission evaluation.
  • Clinically accepts patients or discusses patients with Director of Admissions or other internal Gaylord staff to determine clinical appropriateness.
  • Reviews denials with Director of Admissions, as appropriate.
  • Consults with the Admissions Office on special needs of incoming patients so needs can be appropriately communicated to the receiving Gaylord personnel.
  • Communicates with hospital physicians, care managers, patient and family members regarding admissions process, providing feedback on the status of the referral, answering questions and obtaining any additional information, as indicated.
  • Establishes network of referral sources to increase referrals and admissions.
  • Meets monthly, and annual goals for referral generation and patient admission growth.
  • Conducts in-services and educational rounds to referral sources informing them of Gaylord Hospital services and admission criteria.
  • Performs ongoing needs assessments to identify referral sources' needs for program development.
  • Distributes marketing information to referral sources.
  • Monitors referral trends.
  • Exhibits at pertinent conferences.
  • Documents referrals/denials in Meditech Expanse.
  • Actively participates in Admissions Department operations.
  • Attends and participates in Sales/Admissions staff meetings.
  • Assists in orientation and training of new Regional Healthcare staff.
  • Complies with all Sales/Admissions policies and procedures.
  • Acquires and maintains skills and knowledge regarding Sales/Admissions through participation in relevant educational programs.
  • Acts as an Ambassador for Gaylord Hospital at referral source and in the community.
  • Represents Gaylord at the referring hospital site.
  • First line of contact with prospective patient and family.
  • Maintains Reptrax or Vendermate credentialing as required.
