About The Position

The April Parker Foundation is seeking compassionate, detail-oriented Care Managers (Generalists) to deliver Enhanced Care Management (ECM) and Community Supports (CS) services to Medi-Cal members with complex medical, behavioral, and social needs. Please note: This position requires a monthly IN-PERSON Client visit in your designated area. You’ll work directly with individuals experiencing housing insecurity, chronic illness, or behavioral-health challenges, helping them navigate care, access community resources, and achieve stability in health and housing. This is a field-based / remote role ideal for professionals who value flexibility and meaningful impact. The Regional Care Manager delivers in-person Enhanced Care Management (ECM) and Community Supports (CS) services to Medi-Cal members in their community with complex medical, behavioral, and social needs. This position provides IN-PERSON outreach, assessment, care planning, and ongoing coordination to help members achieve stability in health, housing, and daily living. Regional Care Managers are assigned a territory/region and engage clients in-person to address physical health, behavioral health, and social determinants such as housing, food access, and transportation. Regional Care Managers are compensated through a base hourly wage plus a field performance incentive. The base wage is intentionally set below market rate because the incentive structure is designed to reward and drive in-person service delivery. Care Managers who are consistently in the community serving members earn significantly more. A Care Manager maintaining a caseload of 40–60 members actively receiving in-person services can expect total monthly earnings of approximately $3,800–$5,400, depending on caseload and hours worked.

Requirements

  • Minimum 2 years of experience in case management, care coordination, or related field
  • Knowledge of Medi-Cal CalAIM programs, community resources, and social determinants of health
  • Excellent documentation, organization, and communication skills
  • Valid California Driver’s License, auto insurance, and reliable transportation

Nice To Haves

  • Bachelor’s degree or CHW certification
  • CA licensure (LCSW, LMFT, LPCC, RN, etc.)

Responsibilities

  • Conduct outreach, assessments, and individualized care plans for ECM and CS members.
  • Coordinate medical, behavioral, and social-service supports—including housing navigation and tenancy services.
  • Complete timely documentation and progress tracking in APF systems.
  • Provide in-person, telephonic, and virtual encounters based on member needs.
  • Collaborate with health plans, community partners, and APF multidisciplinary teams.
  • Maintain strict confidentiality and compliance with Medi-Cal, DHCS, and ILS guidelines.
