At FutureCare, our Regional Care Managers (Hospital Liaisons) are an integral part of the admission process, serving as the liaison between the hospitals and our facilities. The Regional Care Manager is responsible for completing the pre-admission screening of patients for appropriateness to FutureCare by reviewing the clinical record, and talking with discharge planners to determine if there is a skilled need. The care manager helps to educate the hospitals to the programs and services we can provide at our various locations. The Regional Care Manager works on-site at the hospital, reviewing referrals, communicating with the discharge planners and other key clinical personnel, the patient and family, and our facility admission departments. The Regional Care Manager is also responsible for building new business from within their hospitals through ongoing business development. Salary Range $88,000 - $97,000 Full Time Annual Salary Disclosure Statement The salary mentioned above reflects the potential base pay range for this role.   • Referral management and clinical review of patients for potential admission
  • Daily presence onsite at hospital key accounts
  • Help to educate referral sources to our programs and clinical capabilities at FutureCare
  • Work directly with all FutureCare locations to direct appropriate referrals for placement
  • Communicate through written and verbal communication with hospital referral sources, patients and families
  • Assist with getting pre-authorization when needed
  • Attend business development meetings at the facilities as needed
  • Participate in weekly census calls as needed
  • Maintain referral logs, and other reports as required
  • RN or LPN with relevant care management/liaison experience preferred, but will consider a bachelor’s degree in a health related field with at least 3 years’ experience as a hospital liaison.
  • Ability to communicate effectively, verbally and in writing, with all level of health care professionals and support staff
  • Ability to maintain confidentiality of resident information in compliance with the prevailing legal requisites
  • Experience with Microsoft Office and Excel software required
  • Must have a valid Maryland Driver License, and be able to drive between hospital accounts
