About The Position

The Regional Care Coordinator is responsible for overseeing, coordinating, and enhancing patient care services across multiple sites within a designated region. This position works collaboratively with local care teams, administrative leadership, and external providers to ensure consistent, high-quality, patient-centered care. The role includes monitoring compliance with care protocols, supporting case management efforts, and driving performance improvement initiatives.

Requirements

  • Must have completed minimum of an associate's degree as a Physical Therapy Assistant, or related field
  • Minimum 3–5 years in physical therapy or adjacent field
  • Strong organizational and time-management skills, experience with scheduling via EMR preferred.
  • Flexibility to adapt to changes in schedules or job priorities
  • Excellent communication and interpersonal skills, written and verbal.
  • Excellent customer service skills.
  • Analytical and problem-solving abilities, ability to manage and prioritize multiple schedules and tasks effectively
  • Proficiency in EMR systems and care coordination platforms

Nice To Haves

  • Bachelor's degree preferred.
  • Experience in a multi-site or regional role preferred.
  • Experience with scheduling via EMR preferred.

Responsibilities

  • Serve as a liaison between regional sites, physicians and other providers, payer partners and corporate care management to ensure alignment of care standards and practices.
  • Coordinate care transitions between payers, hospitals, specialists, and community services to ensure continuity and reduce ER visits and hospital readmissions.
  • Monitor and evaluate regional performance metrics, including utilization, quality indicators, and patient outcomes.
  • Maintain knowledge of payer requirements, community resources, and current healthcare regulations impacting care coordination.
