About The Position
The Community Health Centers is known throughout Vermont for providing compassionate, affordable, quality health care to all who need it regardless of financial status or life circumstance. We are seeking Referrals Representatives to join our CHC family. The Referrals Representative is responsible for the day-to-day processing of all aspects of the patient referral process, and the forwarding of patient records as requested and according to policies and procedures. Location TBD.
