The Marquardt Senior Life Plan Community Skilled Nursing Community Referral Specialist A Senior Life Plan Community is a residential setting that offers older adults a continuum of care – from independent living to assisted living and skilled nursing – within one campus so they can age in place with stability and support. Marquardt Health Center 1020 Hill Street Watertown, WI 53098 We are seeking a dynamic and relationship-driven Referral Specialist to join our team. The Referral Specialist is ultimately responsible for the Nursing Home Census. An individual who thrives on building strong partnerships, communicating with clarity and compassion, and ensuring every referral experience reflects our commitment to person-centered care is best suited for this role. ESSENTIAL JOB FUNCTIONS: · Build and maintain strong connections with hospitals, discharge planners, case managers, off campus Assisted Living facilities, and other community partners seeking solutions for individuals needing post-acute short term or long term nursing home care. · Accurately match potential residents to suitable care levels within The Marquardt Life Plan Community and hand off referrals as appropriate. · Clearly communicate our facility’s clinical capabilities and that Marquardt has an in house, full time, Financial Counselor for our Residents and Families to help them through the complicated Medical Assistance Application and Re-Certification process, as well as assist with other financial or benefit processes. Census Development and Management: · Actively contribute to maintaining and growing skilled nursing census through strategic outreach and timely follow-up. · Monitor referral trends, admission barriers, and market opportunities to support census growth and stability. · Interact and collaborate with internal interdisciplinary team to ensure appropriate and efficient admissions that align with census goals. · Maintain strong relationships with referral sources to support consistent, high-quality referral flow. Requirements QUALIFICATIONS: Commitment to quality outcomes and services for all individuals. Ability to relate well to all individuals. Ability to maintain and protect the confidentiality of information. Ability to exercise independent judgment and make sound decisions. Ability to adapt to change.
