About The Position
The Referral Specialist is responsible for facilitating and managing patient referrals between all HPWO providers/services and outside specialists/agencies. This includes facilitating and managing the summary of care received from the outside specialist/agency, therefore completing the coordination of care for that outside service. This process supports HPWO as the patient's primary care provider and medical home who assumes responsibility of care for the services rendered at outside specialists/agencies.
