As a Referral Services Coordinator, you will be the vital link between patients, families, and clinical teams, ensuring timely and accurate referrals that enable patients to access the right services when they need them most. We are looking for a full-time, 40-hour Referral Coordinator to join our team. This is a hybrid role after training in Newton, MA for the first 60 - 90 days. The hours are 10am - 6:30pm M-F, will include 2 holidays and a weekend rotation. Job Summary Mass General Brigham Home Care provides non-acute, medically necessary skilled care in the home to help “homebound” patients recover from illness or injury and manage exacerbations of chronic disease. Services include skilled nursing care (IV drug administration, injections, tube feeding, wound care, care plan education), physical therapy, occupational therapy, speech language pathology, medical social services, and as needed medical supplies. In this role, you’ll work collaboratively, managing referral workflows, communicating with healthcare providers, patients, and internal teams, and maintaining detailed records that ensure seamless patient transitions. You’ll be supported by an experienced team of nurses, therapists, and care coordinators—all focused on delivering the highest standard of care. Ideal candidates have strong organizational skills, a patient-first mindset, and the ability to multitask effectively in a fast-paced environment. Requirements

  • High School Diploma or Equivalent required
  • scheduling experience in administrative medical office setting 3-5 years required
