Visit us online at https://bhconnections.org Behavioral Health Connections (BHC) in Atlanta, GA is seeking a dynamic and talented Referral Management Specialist Full Time Evenings to work 10 hours shift. The Referral Management Specialist that serves to assess and coordinate the care of patients referred for Mental Health Services. They review referral information, determine the appropriateness of services, and facilitate the connection between patients and the most appropriate facilities or treatment programs. This role is crucial in ensuring patients receive timely and effective care tailored to their specific behavioral health needs.

Requirements

  • Master’s degree in Social Work, Psychology, Counseling, Behavioral Health or related field.
  • Preferred Current license in the State of Georgia as a Licensed Professional Counselor (“LPC”), Licensed Master Social Worker (“LMSW”), Licensed Clinical Social Worker (“LCSW”). RN or LPN
  • Current CPR, Vaccines, if required by a customer hospital. (“Customer hospital” means the hospital to which BHC is contracted to provide mobile assessment services.)
  • Required 1-3 years of work experience working with Assessment and Referral department in a Behavioral Health Setting.
  • Completion of appropriate internship, including the provision of individual, group or family therapy/counseling.

Nice To Haves

  • Preferred 3-5 years of work experience in related field.

Responsibilities

  • Reveiw assessment referrals received and collaborate with RN to assess patient needs.
  • Assist with coordination of patient care
  • Maintain patient recoreds, updating databases, and ensuring accurate documentation of patient information
