About The Position

Behavioral Health Connections, Inc. is a provider of patient-focused, comprehensive mental health services. We offer mental health evaluations to children, adolescents, adults and older adults in crisis 24 hours a day, 7 days a week response, including all holidays. Our service model focuses on the assessment, diagnosis, treatment, and disposition of your mental health patients. Based on the results of an assessment, we determine the most appropriate level of care and recommend treatment facilities. The tele-psychiatric assessment and consultation services we offer are designed to connect people with the care provides best equipped to address their mental health issues. Rapid response means that the patient will get to the most appropriate level of care faster and expedite their journey to wellness. Behavioral Health Connections (BHC) in Dunwoody, GA is seeking a dynamic and talented Referral Management Specialist PRN to work varied shifts. The Referral Management Specialist that serves to assess and coordinate the care of patients referred for Mental Health Services. They review referral information, determine the appropriateness of services, and facilitate the connection between patients and the most appropriate facilities or treatment programs. This role is crucial in ensuring patients receive timely and effective care tailored to their specific behavioral health needs.

Requirements

  • Master's degree in Social Work, Psychology, Counseling, Behavioral Health or related field.
  • 1-3 years of work experience working with Assessment and Referral department in a Behavioral Health Setting.

Nice To Haves

  • Current license in the State of Georgia as a Licensed Professional Counselor (LPC), Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW).
  • RN or LPN.
  • Current CPR, Vaccines, if required by a customer hospital.
  • 3-5 years of work experience in related field.
  • Completion of appropriate internship, including the provision of individual, group or family therapy/counseling.

Responsibilities

  • Review assessment referrals received and collaborate with RN to assess patient needs.
  • Assist with coordination of patient care.
  • Maintain patient records, updating databases, and ensuring accurate documentation of patient information.
