The Referral Coordinator is responsible for ensuring patients receive timely, accurate, and well-coordinated access to care. This position serves as a key liaison among patients, providers, and external facilities by managing referrals, verifying required documentation, and coordinating appointments in compliance with organizational and regulatory standards.

Requirements

  • Strong attention to detail and accuracy
  • Ability to manage multiple referrals simultaneously in a fast-paced environment
  • Professional and effective communication skills with patients and healthcare providers
  • Strong problem-solving, follow-up, and organizational skills
  • Team-oriented with the ability to work independently
  • High School Diploma required
  • Minimum of one year of experience as a medical receptionist or in care coordination required
  • Strong written and verbal communication skills

Nice To Haves

  • Previous customer service experience in a medical setting preferred
  • Epic experience preferred

Responsibilities

  • Review and process incoming referrals to ensure completeness, accuracy, and program eligibility.
  • Verify insurance coverage, authorization requirements, and referral criteria as applicable.
  • Coordinate and schedule patient appointments efficiently to support timely care delivery.
  • Communicate with patients to explain referral processes, appointment details, and next steps.
  • Collaborate with physicians, surgeons, and clinical staff to ensure appropriate patient placement and continuity of care.
  • Communicate with referring providers regarding incomplete or inappropriate referrals, or when patients are unable to be reached.
  • Maintain accurate, up-to-date documentation within the electronic medical record (EMR).
  • Coordinate with external facilities, such as dialysis centers and specialty clinics, to support seamless care transitions.
  • Ensure compliance with organizational policies, HIPAA regulations, and quality standards.
