As a Referral Coordinator you’ll Aid providers with continuity of care by ensuring referrals get submitted promptly to high value specialists and close the loop of care by obtaining consult notes after specialist visits. This is a full-time role involving 8 hours weekday shifts with no weekends schedule. The position works a hybrid schedule with 3 days remote and 2 days onsite. Requirements

Candidates should have a High School Diploma, or associate’s degree in healthcare administration or related field, preferred
Medical terminology knowledge
1-year related experience preferred
Excellent verbal and written communication
Confidentiality of patient information regarding HIPAA laws
Strong computer skills
Strong attention to detail
Team player
Efficient time management and workload prioritization
Strong multitasking skills

Responsibilities

Ensure referrals are submitted, accepted and followed up on in a timely manner
Provide appropriate clinical information to outside entities, as needed
Ensure complete and accurate registration for patients
Assist patients in problem solving issues related to their referrals including taking an advocate role on the patient’s behalf with the carrier to ensure approval of necessary services in a timely fashion
Maintain detailed and accurate referral documentation and communication with referring providers within the electronic medical record
Establish and maintain relationships with providers and outside entities
Review details and expectations about the referral with patients as needed
Ensure patient’s chart is up to date with consult information from other entities
Monitor and ensure proper utilization of the High Value Specialist and Medical Neighborhood networks so patients have access to the right care at the right tim
Communicate effectively with specialist offices assuring safe and coordinated transitions of patient care
