About The Position
The Referral Coordinator in Care Management at Carilion New River Valley Medical Center provides administrative and customer service support to the department, administrative staff, QM and clinical staff, ensuring quality, efficient, and cost-effective services. This role serves as a resource for Senior Director, Unit Directors, Managers, Social Workers, Case Managers, and fellow team members, providing direction, answering questions, and resolving concerns, including research and resolution of issues and requests to ensure timely turnaround. The coordinator is on the front line of the referral process with post-acute providers, responsible for communicating with referral sources and ensuring referrals are processed smoothly and timely. The position involves maintaining databases and files, effectively communicating with the leadership team and staff while keeping files confidential, and utilizing computer skills to create and maintain databases and spreadsheet files, ensuring accuracy, production, and distribution.
