About The Position

PHMC’s Forensic Services department is seeking a full-time Reentry Specialist to support participants in the Delaware County Reentry Project (DCRP). This role is crucial for facilitating successful community reintegration for justice-involved individuals by bridging the gap between incarceration and community life. The Reentry Specialist offers credible, empathetic, and strengths-based support. They work in collaboration with probation officers, the DCRP Case Manager/Behavioral Health Assessor, and Peer Recovery Specialists to develop individualized transition plans. These plans address critical areas such as housing, employment, behavioral health, recovery supports, and compliance with supervision requirements. This is an in-person, field-based position, requiring work in Adult Probation and Parole offices, various community-based settings, and other locations as necessary to support participants.

Requirements

  • High school diploma or GED required
  • At least 2 years of experience working with justice-involved individuals and/or individuals with substance use or mental health disorders
  • Must be able to work a flexible schedule to meet the needs of program participants
  • Strong interpersonal and communication skills with the ability to build rapport with justice-involved individuals
  • Knowledge of behavioral health, substance use disorder treatment, and social service systems in Delaware County
  • Ability to work collaboratively with probation officers, clinicians, peers, and community-based partners
  • Strong organizational skills and ability to manage multiple participants and priorities
  • Ability to document services accurately and timely in an electronic data system
  • Ability to work independently in the field while maintaining accountability to program standards

Responsibilities

  • Complete weekly face-to-face meetings and daily phone contacts with participants to identify strengths and challenges to support their reentry into the community from incarceration.
  • Identify community linkages to support participants’ recovery efforts, including job readiness, educational, vocational, medical, mental, housing, and family support.
  • Make outreach to family members of program participants to assist with Social Determinants of Health (SDOH) concerns that may present as roadblocks to participants’ successful reentry.
  • Provide support to participants to increase reentry success and reduce recidivism.
  • Complete initial outreach (including visits to individuals while incarcerated) and orientation of participants to the program.
  • Provide services in the county jail, homes, treatment facilities, employment locations, and other community-based sites to support participants.
  • Escort participants as needed to appointments for other social, behavioral, or healthcare services to enhance participant compliance with program requirements.
  • Provide education and support in conjunction with the DCRP Case Manager/Behavioral Health Assessor.
  • Provide outreach to participants at treatment facilities in the community.
  • Work as a team with the DCRP Case Manager/Behavioral Health Assessor to ensure consistency of service provision and support to program participants.
  • Attend reentry plan meetings with participants, Probation, and community-based service providers.
  • Provide continuity of care as defined by reentry plans for assigned participants.
  • Assist participants in reentry in areas such as self-esteem enhancements, conflict resolution, assertiveness training, and other recovery skills.
  • Alert the DCRP Case Manager/Behavioral Health Assessor and Supervisor to any current or potential behavioral or health-related problems of participants that may impact program compliance.
  • Provide comprehensive wellness support to participants, including support with accessing routine primary care and specialized health services as identified.
  • Document participant services in the PHMC participant record in accordance with program protocols.
  • Provide on-call/after-hours support on a rotational basis with other members of the PHMC Delaware County Reentry Project team.
