About The Position

The Reentry Services Coordinator at Bay State Community Services Inc. plays a crucial role in supporting individuals transitioning from incarceration back into the community. This position involves conducting outreach to correctional institutions, managing client cases, and facilitating access to various services aimed at aiding successful reintegration. The coordinator will work closely with a multi-disciplinary team to ensure clients receive the necessary support, including education, career counseling, and housing assistance.

Requirements

• Bachelor's Degree in a Human Services related field or Lived-Experience required.
• At least two years of experience working with justice-involved populations.
• Strong computer skills.

Responsibilities

• Conduct in-reach to Massachusetts Correctional Institutions and County House of Corrections to publicize and enroll clients in services.
• Develop rapport with individuals returning to the community to facilitate a 'warm-handoff' process.
• Greet returning individuals at the CJSC to welcome them home from incarceration.
• Create and maintain case files for each client, including demographic information and assessments.
• Manage the caseload of all individuals participating in the RGRSP at the CJSC.
• Coordinate with the CJSC Public Benefits Coordinator to establish essential public benefits for clients.
• Maintain monthly contact with clients for 18 months post-release.
• Provide information about mentoring training to community-based programs.
• Coordinate with community-based mentoring programs to establish connections for clients.
