About The Position

Requirements

Responsibilities

  • Educates, assesses, identifies, and requests appropriate community, government, or private services for clients.
  • Conducts case review meetings and manages case documentation.

  • Establishes and maintains positive relationships with clients, volunteers, mentors, and Reentry team members.
