About The Position

Requirements

• Bachelor's degree in social work, Human Services, or a related field.
• Three (3) years of experience in a social services capacity, particularly with disadvantaged populations in corrections or treatment environments.
• Advanced knowledge of accessing social services and governmental, community, or private human support service agencies.

Responsibilities

• Educates, assesses, identifies, and requests appropriate community, government, or private services for clients.
• Conducts case review meetings and manages case documentation.

• Establishes and maintains positive relationships with clients, volunteers, mentors, and Reentry team members.
