About The Position
The Reentry Facilitator/Care Manager reports to the Assistant Director of Workforce & Career Pathways Development and is responsible for supporting participants referred by County Probation. This includes building and maintaining effective working relationships with probation officers and a network of local service providers. The role involves conducting pre-release screenings, facilitating evidence-based Cognitive Behavioral Intervention (CBI) classes, tracking client progress, and documenting individualized case notes and program outcomes. We are looking for someone who insists on maintaining a positive attitude and enjoys supporting a diverse group of people. This position is full-time temporary, scheduled to run through September 2025.
