The Reentry Coordinator at the Oklahoma County Diversion Hub is responsible for assisting justice-involved individuals transitioning back into the community after incarceration. This role involves assessing client needs, providing case management services, and acting as a liaison between the Oklahoma County Detention Center, Diversion Hub staff, and community agencies. The goal is to facilitate successful reentry by connecting clients with essential resources and support services.

  • Demonstrate the values expected within the program and promote the mission of the organization.
  • Use validated risk needs assessments to create discharge/release plans that address criminogenic risks.
  • Analyze the needs of individuals scheduled for release and create discharge plans to reduce barriers upon release.
  • Meet with individuals in the Oklahoma County Detention Center to assess their needs and gather information for discharge plans.
  • Liaise with the jail, Diversion Hub staff, and community partners to fulfill client needs during their release.
  • Collaborate with criminal legal system stakeholders to ensure clear communication about client circumstances and needs.
  • Review jail release schedules and arrange transportation for reentry to appropriate venues.
  • Streamline client access to needed services to support successful reentry.
  • Evaluate jail-based survey results for reentry program eligibility and make timely referrals to legal system entities or community agencies.
  • Conduct follow-ups on all referrals to confirm approval or denial and utilize other referral options as needed.
  • Prepare documentation for Diversion Hub staff to assist with emergency needs and inform case managers of action steps.
  • Maintain a database to track client information, referrals, outcomes, and meeting notes.
  • Maintain communication with the public defender's office and Diversion Hub staff to meet legal requirements upon release.
  • Collaborate with pre-trial bond and diversion programs to stay updated on necessary processes.
  • Serve as a liaison to partner agencies, attorneys, law enforcement, and referring sources.
  • Maintain communication with Justice Navigators, onsite service navigators, and clients.
  • Adhere to agency code of ethics and professional standards, including client confidentiality.
  • Attend staff development, training, and meetings as required.
  • Perform any other duties within the scope, spirit, and purpose of the job.
  • 1 to 3+ years of experience in social work, communication, criminal justice, political science, or a similar human service area.
  • Experience in case management preferred.
  • Valid driver's license and insurance on motor vehicle in accordance with state law and agency requirements.
  • Clearance of background check.
  • A working knowledge of the criminal justice system.
  • Excellent oral and written communication skills.
  • Ability to interact with diverse cultural and socioeconomic backgrounds.
  • Capacity for teaching adults and older individuals.
  • Proven track record in developing community partnerships and collaborations.
  • Ability to meet multiple deadlines and balance numerous projects.
  • Independence and self-confidence to act decisively while integrating others' ideas.
  • Positive, mission-driven, and extroverted work style.
