The Re-Entry Case Management Specialist is responsible for: Assessing and evaluating offender needs using designated assessment and risk-assessment instruments. Monitoring offender compliance with all programs and ECSD rules and regulations. Facilitating evidence-based psychoeducational groups for assigned population as defined by the curriculum and according to Spectrum standards. Providing reentry planning for eligible pretrial inmates. Facilitating reentry/discharge planning for assigned sentenced populations. Completing Strengths, Needs, and Culture Discovery. Reentry data/progress transfer within continuum (Middleton, ECPRC, WIT, STAR). Providing support and guidance for inmate population to navigate and addressing needs/issues (Canteen/Inmate Accounts, Property, Visits, Notary service, grievances, and facility processes).
