This role focuses on referral coordination, patient navigation, clinic flow support, and data tracking to promote same-day access to antiretroviral therapy (ART), retention in care, and viral suppression outcomes. The position supports daily clinic readiness, patient outreach, and grant-related administrative functions in alignment with quality management and funding requirements. Supervisory Accountability Involves no responsibility or authority for the direction of others. Nature and Scope External contacts include a variety of third party payers, medical organizations, patients and other healthcare personnel

Requirements

  • High school diploma or equivalent required; Associate degree preferred.
  • Minimum 1–2 years of experience in healthcare, public health, case coordination, or clinic operations preferred.
  • Experience in HIV care or infectious disease settings strongly preferred.
  • Proficiency in electronic medical record (EMR) systems required.
  • Basic Life Support (BLS) certification preferred or obtained within organizational timeframe (if required by organization).

Nice To Haves

  • Knowledge of HIV treatment models, Rapid ART workflows, and linkage-to-care best practices preferred.
  • Understanding of HIPAA and patient confidentiality regulations.
  • Ability to manage time-sensitive workflows and prioritize urgent linkage activities.

Responsibilities

  • Coordinate same-day or expedited medical appointments, laboratory services, and provider consultations.
  • Facilitate timely initiation of ART in collaboration with providers, pharmacies, and laboratories.
  • Track linkage to care within 72 hours and document services in the electronic medical record (EMR).
  • Monitor retention and viral suppression indicators to support continuity of care.
  • Communicate with interdisciplinary team members to ensure seamless transitions into care.
  • Receive, process, and track internal and external referrals for HIV primary care and Rapid ART services.
  • Monitor referral outcomes and conduct follow-up on incomplete, missed, or delayed appointments.
  • Conduct outreach calls to support re-engagement efforts and reduce gaps in care.
  • Identify and escalate barriers to care (e.g., transportation, insurance status, medication access) to appropriate team members.
  • Coordinate warm handoffs to internal departments and community-based support services.
  • Prepare clinic areas daily to ensure operational readiness, including exam room preparation and supply inventory monitoring.
  • Provide patient check-in and check-out support as needed; serve as backup to front desk staff.
  • Conduct appointment reminder calls, lab reminders, and follow-up communications.
  • Support medication refill coordination by processing refill requests and communicating with providers and pharmacies.
  • Assist with patient flow and triage by identifying urgent concerns and notifying licensed clinical staff appropriately.
  • Maintain accurate, timely documentation of coordination activities in the EMR.
  • Support infection control practices and clinic safety procedures.
  • Track and report referral-related performance measures, including linkage to care, retention in care, and viral suppression.
  • Maintain tracking logs and data systems related to Rapid ART and Red Carpet initiatives.
  • Support quality management initiatives and continuous improvement efforts.
  • Assist with grant administrative tasks, documentation, and reporting requirements to ensure compliance with funding benchmarks.
  • Generate reports and maintain supporting documentation for audits and program monitoring.
