About The Position

RESPONSIBLE FOR CONDUCTING ALL INITIAL ASSESSMENTS, SCHEDULED REASSESSMENTS, SIGNIFICANT CHANGE IN CONDITION AND OTHER ASSESSMENTS IN A HOME AND COMMUNITY-BASED MODEL OF CARE—COMPLETING THEM PRIOR TO SCHEDULED TEAM CONFERENCES. COMMUNICATES PARTICIPANT CHANGES TO TEAM MEMBERS. DEVELOPS AND IMPLEMENTS THERAPEUTIC SMALL GROUP ACTIVITIES BASED ON PARTICIPANT NEED AND INPUT FROM THE INTERDISCIPLINARY TEAM (IDT)/CARE TEAM AND DAY CENTER STAFF. REPORTS DIRECTLY TO CENTER MANAGER. This role requires a BS degree in Recreation Therapy and a minimum of 1 year of experience in a social or recreational program providing and coordinating services for a frail elderly population within the past 5 years. A Licensed Recreational Therapist in NC is required. The individual must be able to work effectively in a team environment, have a good understanding of psychosocial problems of aging, and only act within the scope of his or her authority to practice. They must also meet a standardized set of competencies for the specific position description established by Senior Total Life Care and approved by CMS before working independently. Immunizations must be up to date, and the individual must be medically cleared for communicable diseases before engaging in direct participant contact. The position involves rendering care to Geriatric patients.

Requirements

  • BS degree in Recreation Therapy

Responsibilities

  • Conducts all initial assessments and scheduled reassessments of all participants, or with a significant change in condition, or as requested by the IDT/Care Team. Completes assessments prior to scheduled team conference and communicates participant changes to team members.
  • Records, signs, and dates in the participant’s medical record in a timely manner as outlined in the STLC Documentation policy.
  • Plans, develops & implements specific programs/groups as part of the plan of care for therapeutic recreational activities.
  • Participates as a member of the IDT/Care Team. Attends staff meetings and takes part in participant care planning. Communicates any changes in participant condition to team members.
  • Plans a monthly/daily calendar for the Adult Day Health Center (ADHC)/Social Center based on participant interest and in collaboration with the center CNAs.
  • Serves as a resource to staff.
  • Participates in Quality activities as appropriate.
  • Coordinates with various community resources as appropriate.
  • Supports team members and functions as a team player while providing that support and adheres to the “do no harm” team contract statement.
  • Supports Senior TLC’s mission to encourage and support the quality of life of seniors wishing to continue living in the community; its vision to be the preferred provider of individualized care for seniors in the community; and its values of respect, integrity, accountability, compatible goals, and compassionate care.
  • Other duties as assigned.
