As an LPN, you will play a key role in administering medication, performing assessments, and maintaining accurate medical records. Your contributions will enhance the quality of care we provide and help our clients navigate their recovery journeys.

  • Administer medications to clients through various methods, including oral, injection, and self-administration, based on service line requirements.
  • Perform nursing assessments, including physical and mental health assessments and psychiatric screeners, in accordance with service line specifications.
  • Maintain accurate documentation of medication administration and nursing assessments, ensuring compliance with service line standards.
  • Conduct medication administration record (MAR) reviews and monitor medication stock for housing clients.
  • Communicate effectively with clients regarding medication inquiries and manage refill requests with providers.
  • Verify client data and medical history during sessions, obtaining necessary preliminary health information.
  • Collaborate in timely documentation of nursing assessments, interventions, and educational groups.
  • Function as a resource for nursing and medical inquiries, overseeing client health issues while working under nursing leadership.
  • Participate in all required meetings and perform additional duties as required by nursing leadership.
  • Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
  • Graduation from an accredited practical nurse program.
  • Must successfully complete background checks including criminal, driving, and abuse/neglect credentials.
  • Current driver's license, acceptable driving record, and auto insurance required.
