About The Position

The REACH Coordinator works with individuals with developmental disabilities and behavioral health needs, their families/care givers, and support providers, and community partners to strengthen treatment/support efficiencies and service outcomes. The REACH Coordinator serves as a vital community education resource to the community and coordinates admission into REACH services.

Requirements

• Graduation from an accredited university with at least a Master's Degree in a Human Services related field.
• Two years of experience working with individuals with developmental disabilities and behavioral health.
• OR a Bachelor's Degree and five years of experience working with individuals with developmental disabilities and behavioral health diagnoses is required.
• QMHP is required.
