The County of San Mateo Human Services Agency is seeking qualified candidates for the Re-entry Social Worker I/II/III position. This role is situated within the Employment Services division of the Human Services Agency, specifically at Service Connect, which is San Mateo County's multidisciplinary re-entry program. This program is designed to support high-risk individuals who have recently been released from prison or jail, assisting them in reintegrating back into the community. The Re-entry Social Worker I/II/III will be responsible for providing comprehensive case management services to adults and their families, focusing on ongoing support, stabilization, and the removal of barriers to successful employment and re-entry following incarceration. In this position, the Re-entry Social Worker will conduct thorough needs assessments to identify service areas and will collaborate as part of an interdepartmental team. The role requires a significant degree of independence and initiative, as the social worker will be expected to conduct comprehensive assessments at intake and provide ongoing case management activities for participants. Responsibilities include conducting psychosocial needs assessments, engaging in multidisciplinary team meetings, completing necessary documentation, developing casework plans, and providing skilled casework services to clients and their families. Additionally, the social worker will assist clients in navigating subsidized employment programs and addressing behavioral barriers to employment, interpret relevant regulations, and respond to various client needs, including financial, housing, legal, and emotional crises. The ideal candidate will possess a Master's Degree in Social Work and have experience with the unique needs of the adult re-entry population, particularly regarding barriers to employment. Strong decision-making, client engagement, crisis management, and advocacy skills are essential, along with superior organizational abilities to manage a large caseload. Proficiency in computer skills, particularly with database systems and Microsoft Office applications, is also required. Excellent writing and documentation skills are necessary, as is the ability to work collaboratively within a Multi-Disciplinary Team, coordinating services with partner agencies.
