The Case Manager is responsible for the casework of New Roads clients. This role involves developing transitional living plans, assessing clients' needs, coordinating with community resources, and providing ongoing support to at-risk youth.
Develops written transitional living plan with each youth.
Assesses client's service and housing needs.
Coordinates plan with other community resources.
Evaluates progress with the youth weekly.
Provides backup case management services for co-workers as assigned.
Reviews progress with Program Supervisor and New Roads team weekly.
Completes required documentation of client activities and progress as needed.
Coordinates and assists in housing opportunities for youth.
Develops housing plan with individual youth.
Secures housing in conjunction with New Roads youth.
Makes weekly visits to youth in housing.
Provides aftercare services to youth.
Acts as positive role model.
Completes other duties as assigned.
Casework experience and one year working with runaway, homeless, or at-risk youth preferred.
Valid Oregon Driver's License, acceptable record, and insurability under Agency's auto insurance policy.
Good communication skills.
Ability to work effectively as a member of an integrated team.
Ability to relate effectively with youth, parents, and community.
Ability to work with people of diverse cultural backgrounds.
Ability to maintain confidentiality.
Ability to work in an unstructured, sometimes stressful environment, and maintain a positive mental attitude.
