Job Summary and Qualifications The Radiology Nurse Navigator is responsible for all departmental functions in support of the hospital mission, vision and facility goals. This position establishes effective working relationships with all constituencies including patients, physicians, employees, and volunteers. The Radiology Nurse Navigator will perform direct patient care within the scope of the Registered Nurse Practice Act, with emphasis on the patient receiving invasive, diagnostic and interventional procedures. The duties include coordinating the activities of the Cardiac CT Program in collaboration with the Radiology, Cath Lab, and Cardiology departments. The Radiology Nurse Navigator assists the department Director in recommending and evaluation of operational efficiencies and process modifications in meeting facility goals. The Radiology Nurse Navigator identifies and remedies through continuous customer satisfaction data and employee involvement. This role will start with the time of the patients diagnosis and will continue through the patients journey throughout their care, will be the patient advocate and care for them in a highly supportive and personal manner and also will serve as a liaison to ensure that patients and families receive resources and services as needed. The Radiology Nurse Navigator will provide high quality care including:

Requirements

  • Advanced Cardiac Life Spt must be obtained within 30 days of employment start date
  • Basic Cardiac Life Support must be obtained within 30 days of employment start date
  • (RN) Registered Nurse

Nice To Haves

  • Two years oncology n u rsing experience .
  • Bachelor of Science in Nursing and Oncology Nurse Certification

Responsibilities

  • Coordinating activities of coronary CTA patients
  • Provides language specific education, psychosocial support, and necessary resources for newly diagnosed patients pertinent to their specific disease, and their families (language specific), and coordinates appropriate referrals for those needed services.
  • Educates and provides information to new and existing patients and families regarding diagnosis; work-up; treatments to include surgery, radiation therapy and chemotherapy; support services, financial issues; community resources; survivorship issues and follow-up.
  • Serves as liaison for patients and families between departments.
  • Ensures and coordinates (where appropriate) new patient referrals, scheduling of exams, procedures and appointments, identifies potential gaps as well as the coordination of care for patients returning to their communities for treatment and follow-up.
  • Contact every newly diagnosed patient at the time of the diagnosis.
  • Assesses every new patient for emotional and social needs as well as barriers to care such as: Health insurance, transportation, etc., and refers to the needed resources.
  • Identifies trend barriers to patient expedient care and will conduct service recovery as needed. Responsible for patient satisfaction.
  • Assesses and helps coordinate physician scheduling to ensure appropriate coverage of CT needs.
  • Identifies patients that need scheduling at specific location given case complexity.
