ESSENTIAL FUNCTIONS Knows, understands, incorporates and demonstrates the mission, vision and values of Trinity Health in leadership, behaviors, practices and decisions. Tracks and documents interventions and outcomes. Ensures appropriate data are collected to track program interventions and outcomes. Ensures appropriate and timely communication of patient progress to referring physicians and other care providers. Coordinates scheduling/referring of patient to specialty providers throughout the healthcare system. Prepares patients for appointments, providing information in a clear understandable manner and providing written follow-up in the manner patient prefers (text, email, mail etc.) Serves as a direct contact and resource for patient and family, providing emotional support and education. Acts as a resource for patient scheduling, clinical resource management, patient access and others as necessary to ensure appropriate patient access and follow-up. Demonstrates excellent critical thinking skills as they interview patients and gather information to assure accurate and timely assessment and intake into the program, entering information into all computer systems necessary. Reviews radiology reports. Compiles and reviews information contained within reports to assist and support the following of algorithms, generation of documents, and formalized processes for transition in commonly followed care pathways. Attends care conferences, tumor boards and other meetings as necessary. Ensures that appropriate patient data are available for conference. Collaborates with program stakeholders in support of data collection for registry participation, quality reporting, etc. Collaborates on performance improvement activities as indicated by outcomes in program efficiency, patient experience and outcomes. Provides feedback instrumental to current program change to support ongoing improvement of the program model. Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution. Maintains the confidentiality of information acquired pertaining to patient, physicians, colleagues, and visitors to Trinity Health. Discusses patient and hospital information only among appropriate personnel in appropriately private places. Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management. Performs other duties as assigned. Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health Integrity and Compliance Program and Code of Conduct, as well as other policies and procedures to ensure adherence in a manner that reflects honest, ethical, and professional behavior.
