About The Position

The Quality Program Analyst-Care at Home plays a crucial role in maintaining and enhancing the quality of care provided to patients. Under the supervision of the Quality Coordinator and reporting directly to the Assistant Area Administrator, this position is responsible for overseeing ongoing quality and performance improvement projects. The analyst will actively participate in risk management initiatives and identify patterns and trends in patient care problems, ensuring compliance with Quality Improvement requirements. This role involves assisting in the design and implementation of solutions to address quality management issues, thereby contributing to the overall improvement of patient care services. In addition to these responsibilities, the Quality Program Analyst will integrate various data reporting systems to develop and maintain a range of statistical reports. These reports are essential for clinicians, leaders, and relevant groups to visualize variations in practice patterns. The analyst will also be tasked with obtaining consensus for the revision or termination of reports, coordinating data collection procedures, and developing quality control and improvement indicators. This includes establishing data sources and methods for collection, display, and analysis, ensuring that all activities align with federal, state, and local standards. The role requires close collaboration with the leadership team to compile clinician-specific and team-specific data, which supports leaders in recognizing strengths and weaknesses within their teams. The analyst will also conduct concurrent and retrospective reviews using approved indicators, ensuring that quality improvement activities meet organizational guidelines and regulatory requirements. Overall, this position is integral to fostering a culture of continuous improvement and excellence in patient care within the organization.
