If you would like the genuine opportunity to make a profound difference in the delivery of primary care and community health, we invite your interest and application after reviewing the specifics and requirements for the Quality Nurse Manager listed below POSITION SUMMARY The Quality Nurse Manager is a senior clinical leadership role within the Brevard Health Alliance (BHA) Quality Department. Reporting directly to the Director of Quality, this position is responsible for leading the quality nursing team, overseeing the Advanced Care+ (AC+) senior care concierge program, and managing a portfolio of care management services including Transitional Care Management (TCM), Chronic Care Management (CCM), and Annual Wellness Visits (AWVs). The Quality Nurse Manager serves as a key driver of clinical quality improvement, population health outcomes, and care coordination excellence across BHA's patient panels..

Requirements

  • Active, unrestricted Florida Registered Nurse (RN) license required.
  • Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) strongly preferred.
  • Minimum 3–5 years of clinical nursing experience required; primary care or community health setting preferred.
  • Minimum 2 years of nursing supervisory or management experience required.
  • Experience with care management programs (e.g., TCM, CCM, AWV) or similar value-based service lines strongly preferred.
  • Experience in a Federally Qualified Health Center (FQHC) or similar safety-net environment preferred.
  • Familiarity with UDS, HEDIS, or other health quality measures preferred.

Nice To Haves

  • Master of Science in Nursing (MSN) or advanced certification in care management or quality is a plus.

Responsibilities

  • Provide direct supervision, mentorship, and performance management for BHA quality nurses.
  • Assign and monitor quality nursing workflows, caseloads, and productivity benchmarks.
  • Facilitate regular team meetings, huddles, and professional development activities.
  • Collaborate with the Director of Quality to align nursing activities with organizational quality goals, UDS measures, and HEDIS benchmarks.
  • Serve as a clinical resource and escalation point for complex patient care coordination issues.
  • Oversee the daily operations, staffing, and strategic development of assigned care programs, including but not limited to the Advanced Care+ (AC+) senior care concierge program and BHA's care management service lines.
  • Serve as direct supervisor for program staff, including hiring, onboarding, scheduling, and performance evaluation.
  • Ensure program delivery is person-centered, equitable, and aligned with evidence-based standards and organizational priorities.
  • Monitor program performance against quality, compliance, and productivity targets; identify gaps and implement corrective strategies proactively.
  • Ensure accurate and compliant documentation and billing practices across all program areas, in collaboration with finance and coding teams.
  • Track and report program metrics, patient outcomes, and staff performance to the Director of Quality on a regular basis.
  • Identify opportunities to expand, enhance, or develop programs in response to patient population needs, payer requirements, and quality benchmarks.
  • Support ongoing quality improvement initiatives and care gap closure activities aligned with UDS, HEDIS, and AAAHC standards.
  • Contribute to data review, root cause analysis, and corrective action planning related to nursing and care management workflows.
  • Participate in quality committee meetings and department-wide performance reviews.
  • Assist with patient safety events, incident review, and implementation of process improvements as needed.
  • Partner with analytics, behavioral health, finance, and operations teams to coordinate care and optimize patient outcomes.
  • Serve as a liaison between the Quality Department and clinical leadership across BHA's clinic locations.
  • Support staff education efforts related to quality metrics, documentation standards, and care management best practices.
