About The Position

The Quality Measure Nurse at Venza Care Management LLC is responsible for monitoring, analyzing, and enhancing quality measures within a skilled nursing facility. This role focuses on ensuring compliance with regulatory standards, tracking key performance indicators, and driving quality improvement initiatives to elevate resident care and safety. The position allows for a significant amount of remote work, with occasional travel for meetings and training.

Requirements

• Must be a Registered Nurse (RN) with an active compact state license.
• Minimum of 2 years in a skilled nursing facility or healthcare quality role, with strong experience in QAPI and CMS quality measures.
• Proficient in quality assurance methods, data analysis, and quality improvement techniques.
• Excellent attention to detail and communication skills.

Responsibilities

• Track and analyze key performance indicators like rehospitalization rates, fall incidents, pain management, and infection control.
• Prepare reports and present data-driven insights to the Quality Assurance and Performance Improvement (QAPI) Committee.
• Lead and support initiatives that enhance clinical outcomes and resident experiences using evidence-based practices.
• Ensure adherence to state, federal, and CMS quality standards, assist with audits and inspections, and implement corrective actions as needed.
• Develop and deliver training on quality measures, documentation best practices, and QAPI initiatives.
• Coach staff to ensure accurate data collection and quality-focused care.
• Participate in incident investigations, risk assessments, and the development of safety protocols to prevent adverse events.
• Collaborate with interdisciplinary teams to align quality efforts with facility goals.
