About The Position

We are a 14 bed intermediate care facility creating a beautiful home for medically fragile children and adults with significant cognitive and medical needs. POSITION SUMMARY: The QIDP is responsible for the habilitation training of residents serviced under the continuous active treatment program. The QIDP manages and coordinates the overall development and implementation of client-based habilitation programming/individual program plans (IPPs) for all residents. The QIDP coordinates all internal and external services in order to meet resident needs and ensure quality care and ensures all services are therapeutic and compliant with the IPP and licensing rules. (i.e. speech, medical, pharmacy, social services, psychology, psychiatry, vocational, recreational, educational, occupational therapy, physical therapy, dental and dietary).

Requirements

Valid State of Maine driver license, reliable transportation with proof of registration and insurance, and willingness to use own transportation for work.
Bachelor’s Degree in a human services field including but not limited to: sociology, special education, rehabilitation counseling, and psychology or another qualified healthcare professional with qualified license/registration/certification as specified in title 42 CFR 483.430 paragraph (B)(5).
One-year experience working directly with people with intellectual disabilities.
Two years’ supervisory experience.

Responsibilities

Oversee the Individual Program Plan (IPP) and the Interdisciplinary Team process for each client upon admission; ensure that meetings are scheduled and facilitated and plans are written and distributed in a timely manner, within the first thirty (30) days after admission and reviewed at least monthly, thereafter.
Coordinate programming services of all departments and assure the delivery of active treatment for each client.
Ensure the implementation and monitoring of behavior programs in accordance with the Maine State Behavior Regulations and the ICF/IDD licensing regulations.
Ensure the timely arrangement for all services to be provided by other consultants and service providers as recommended in the Plan, in coordination with any others who were assigned the responsibility.
Ensure that services at school and day programs are provide as required in the person’s IPP and document, at least monthly.
Make recommendations regarding each person’s individual plans and behavior management programs to the supervisor of the day or school programs, if necessary.
Meet with each client’s day or school program supervisor monthly ensuring plans are in place and being followed.
Develop, implement, monitor and assess programs for each person’s IPP as set forth by the Team, based on complete, relevant, diagnostic and prognostic data.
Review records on person’s program, and develop a report at least monthly, to be filed in the person’s individual record.
Modify objectives and methodology in individual plans and/or behavior management program when the person is not demonstrating sufficient progress toward behavioral objectives as recommended by the Team.
Serve as a member of the admission/discharge review team.
Instruct each employee in the proper method of implementation of plans and behavior management programs.
Observe, document and evaluate individual plans and behavior management programs being implemented by each employee.
Directly supervises Developmental Training Coordinator and Developmental Trainers.
Ensures DT-C, DTs and DTAs are trained in the effective delivery of IPP activities and ensures compliance with habilitation plans.
Hold staff accountable to compliance with all state and federal regulations and licensure requirements
Serve as a member of the administration leadership team.
Complete documentation and writing responsibilities in a timely, accurate, actual, objective, detailed, and neat manner using correct spelling and grammar.
Review, sign, and document appropriate action on all reports in a timely manner.
Maintain up-to-date knowledge in field, actively participating in professional development, in-services, training and conferences to enhance knowledge and skills.
Must be able to squat, stoop/bend, kneel, climb stairs, lift at least 20 pounds, and push and pull against potential resistance.
Adequate vision, reading, writing and documentation skills, and hearing to perform the essential functions of the position.
Ability to maintain assigned work hours, having sufficient endurance to perform tasks over extended periods.
