The Quality Improvement Specialist provides communication, reporting, technical, and administrative support to the Quality department at CHCN. The Quality department works to enhance population health and quality improvement within the network, and to facilitate care improvement and efficiency at our member health centers. This role will work closely with member health centers on clinical quality improvement projects to support success in quality outcomes and incentive programs. This will include coordination of regular meetings and follow up, working with our analytics team to share valuable quality data and reports, and collaboration with quality staff and leadership to develop interventions for improvement. The role will also interface with health plan quality staff and other internal and external partners. The ideal candidate is a well-organized, flexible, team player with a passion for data and improving health outcomes.
