About The Position

The Quality Improvement Specialist at Constellation Quality Health is responsible for providing technical assistance and implementation support to healthcare providers, including hospitals, nursing homes, and outpatient practices. This role focuses on improving patient care and outcomes through quality improvement initiatives across a multi-state region. The specialist will engage with healthcare providers, conduct assessments, develop Quality Action Plans, and deliver customized technical assistance based on data analysis and quality improvement coaching.

Requirements

  • Bachelor's degree in healthcare administration, nursing, public health, or related field plus 3+ years of relevant experience in healthcare quality improvement, or a master's degree without experience.
  • CPHQ or Six Sigma Yellow Belt certification is desirable.
  • Clinical credentials or licensure (e.g., registered nurse, social work, nursing home administrator) are a plus but not required.
  • Demonstrated ability to work collaboratively as part of a multidisciplinary team.
  • Excellent interpersonal communication and teamwork skills.
  • Strong oral and written communication skills, including experience in making presentations and conducting training sessions.

Responsibilities

  • Delivers technical assistance to assigned health care providers in specific settings (nursing home, hospital, outpatient practice).
  • Participates in FQS as assigned based on their setting of care and geographical location.
  • Supports the coordination, complementing, or creation of activities arising from resource assessments (A3C).
  • Conducts assessments and develops Quality Action Plans (QAP).
  • Delivers customized technical assistance based on QAP, including data review and quality improvement coaching.
  • Records all QI data and information into the CMS-designated data system and HQI CRM accurately and promptly.
  • Interprets data on clinical measures to guide recommendations for improving results.
  • Monitors and maintains engagement of assigned providers.
