About The Position

As we continue to provide world class primary care services to our enterprise clients, we are growing the team that is dedicated to developing and implementing innovative programs that enhance health outcomes for employees of our current and future enterprise clients. We are seeking to hire a Quality Improvement Program Manager to join our growing Employer Strategic Programs team. You will be part of a team responsible for demonstrating our value proposition of better health at a lower cost of care, which is essential for serving existing clients and for supporting growth. You will have the opportunity to provide administrative and clinical quality improvement support across multiple clinical programs and services in the employer space, extending beyond care delivered onsite, near site and into remote care delivery as well. In this role, you will readily take on the day-to-day tasks to drive program improvement, consistently demonstrating a can-do attitude, while stepping up to address logistical issues. It is critical that you are detail-oriented, highly organized, and quick to learn. You will prioritize accountability, holding yourself and others responsible for meeting commitments and designing feedback loops into work. You will also have an interest or background in project management and enjoy working cross-functionally.

Requirements

  • 1+ year experience in healthcare Quality/HEDIS data medical record review and abstraction
  • 1+ years of experience in navigating and working in an electronic medical record system (e.g. Epic, 1Life, Athena)
  • 1+ years of primary healthcare or outpatient setting experience
  • Intermediate knowledge and understanding of HEDIS related improvement projects

Responsibilities

  • Partake in project management and standard workflow efforts for live and pilot programs for Employers (i.e. multidisciplinary group programs, chronic condition management, mental health programs, etc.)
  • Troubleshoot issues related to these programs and triage questions from patients, providers and care teams
  • Serve as a liaison between the central team and local teams; provide feedback to continuously improve processes, communication and collaboration
  • Perform regular program support and coordination responsibilities including but not limited to documentation creation and management, quality improvement initiatives, data and outcomes tracking and reporting at a regular cadence, patient journey coordination, implementation support, quality assurance support, and operational workflow support
  • Experience analyzing metrics to implement business improvements
